PM-TOM facilitates the optimization of polypharmacy treatments for 60 prevalent diseases by analyzing 615 medications and 450 adverse drug reactions (ADRs). Given a wide range of available pharmacological options, PM-TOM systematically selects medications that minimize ADRs and drug interactions, promoting safer treatment strategies.
The optimization report provides a detailed analysis of drug-drug interactions (DDIs), drug-condition interactions (DCIs), drug-food interactions (DFIs), and drug-gene interactions (DGIs), while also identifying potential risks according to the Beers criteria at both the individual drug and interaction levels. Additionally, the system detects possible prescription cascades. The Examples section includes preset data for two polypharmacy treatment scenarios, offering a structured introduction to the application’s functionalities.
Furthermore, the system integrates ChatGPT support, allowing users to explore alternative treatment options based on existing ADRs, examine pharmacokinetic and pharmacodynamic properties, and refine treatment plans accordingly.
PM-TOM enables the development of multiple treatment versions for polypharmacy management. This functionality facilitates the direct comparison of selected medications and their respective interactions, supporting well-informed decision-making regarding treatment selection and modification.
3. Create a Deprescribing Plan with AI Support
Deprescribing is a critical process aimed at adjusting, reducing, or discontinuing medications to improve treatment safety and patient outcomes in cases of polypharmacy. Through ChatGPT-assisted analysis, PM-TOM can generate structured deprescribing plans, ensuring a systematic transition between medication regimens while maintaining therapeutic efficacy.
